Eastern Highlands Health District Community Survey

Eastern Highlands Health District wants your input! They are conducting an anonymous community assessment during the month of April to better understand local barriers to receiving flu and COVID-19 vaccines. One of the tools they are using is a community survey; they need all perspectives represented in the survey so please participate. The survey will be open through April 21st.
